Ryan, I agree that the examples you provided are a long way to not have a crosswalk.
I don't think the question that you pose at the end of your post is as black and white as you make it sound. Most of the time, people won't have to go one inch out of their way to cross at a crosswalk. You make it sound like every time there is a gap of more than 200m between intersections, the majority of pedestrians are forced to make some sort of critical decision between risking their life and walking a half km. It seems a little over dramatic.
